SGA Anonymous Complaint Form

General Information

Purpose
The purpose of this form is to submit an anonymous complaint relating to SGA or something that is occurring within SGA. This form is reviewed by the Judicial Branch, so if you have a complaint about a justice, please submit a complaint through the Executive Branch's form. In your submission, provide as much detailed information as possible.

This form is not the place to submit any Incidents of Bias or Sexual Misconduct.
